SEDS-USA needs to have updated information on how to contact your chapter. So we ask each chapter to submit this form. The deadlines for this form are January 31 and October1. You do NOT need to submit this form twice per year. Please choose the deadline that is the soonest after your chapter's yearly elections (e.g. chapters that hold elections at the beginning of the calendar year submit this form by January 31st, and chapters that hold elections at the beginning or end of the academic year submit this form by October 1st).
